Those of you applying for a student internship should complete the information from "Student's name" to "Campus phone" online; print the entire document and take it with you to your internship/practicum interview with your potential supervisor. The supervisor should complete the form and sign it. You should then sign the form and make an appointment with the Internship Coordinator to turn it in.
Student Intern Candidates should complete this form online, print it and bring it to the first appointment with the Internship Coordinator.
Here are some examples of logs which the student intern will be writing and submitting via e-mail once a week after the internship/practicum has begun.
